Perimenopause does not begin on one specific day, and menopause does not mean that everything changes overnight.

For some women, the first signs are irregular periods. It can begin several years beforehand, as periods and symptoms start to change, and ends one year after the final menstrual period.

Menopause is confirmed retrospectively after 12 consecutive months without a period, when there is no other explanation for the absence of menstruation. For most women, it occurs between the ages of 45 and 55. [1,3]

Symptoms vary considerably from one woman to another and may include:

hot flushes and night sweats;

sleep problems;

mood changes;

irregular periods during perimenopause;

vaginal dryness;

joint or muscle discomfort;

changes in body composition.

You do not have to experience all of them. Some women go through this stage with relatively few symptoms, while for others the symptoms can have a significant impact on everyday life. [1–3]

What should nutrition look like during perimenopause and menopause?

There is no single food that can “balance your hormones”, and there is no special diet that can stop menopause.

A more useful goal is to build a way of eating that supports your long-term health.

As a simple structure, try to include regularly:

a source of protein;

vegetables and fruit;

whole grains, potatoes or pulses;

sources of unsaturated fats, such as olive oil, nuts, seeds or fish.

A Mediterranean-style eating pattern — rich in plant foods, fish, pulses, whole grains and predominantly unsaturated fats — is associated with benefits for cardiometabolic health. Intervention studies in postmenopausal women suggest improvements in some measures such as blood pressure and blood lipids, but this does not mean that the Mediterranean diet is a “treatment for menopause”. [4]

In other words, you do not need an exotic diet. You need a structure you can maintain.

If it feels easier to start with an organised menu, Hormonal Balance is a 14-day meal plan designed for the perimenopause and menopause stage. It does not treat symptoms or “balance hormones”; it provides a practical structure for everyday meals.

Protein and muscle mass become more important

Around menopause, attention should not be focused only on the number on the scale.

With ageing and the menopausal transition, changes in muscle mass and strength may occur, alongside a tendency for more body fat to be stored around the abdominal area. However, the exact contribution of menopause, ageing, physical activity and energy balance varies from one person to another. [5]

This is why it is useful to include a regular source of protein in your main meals, such as:

fish;

eggs;

Greek yoghurt, fresh cheese or other dairy products;

lean meat;

tofu and other soy foods;

lentils, chickpeas and beans.

You do not need to turn every meal into a “high-protein meal”. What matters more is that protein is not consistently missing from your diet.

And food is only half of the picture.

Resistance exercise — using weights, resistance bands or your own body weight — is important for maintaining muscle mass and strength. Current guidelines specifically recommend maintaining muscle strength and mass through physical activity. [2,6]

What matters for bone health?

After menopause, declining oestrogen levels contribute to loss of bone density and an increased risk of osteoporosis and fractures. [1,7]

Two important nutrients are calcium and vitamin D.

Food sources of calcium can include:

milk, yoghurt and cheese;

fortified plant-based drinks;

some types of tofu;

sardines eaten with their bones;

certain green vegetables;

fortified foods.

Vitamin D is more difficult to obtain entirely from food, and individual needs and recommendations about supplementation depend on several factors.

This does not mean that every woman going through menopause should automatically start taking calcium and vitamin D supplements. Dietary intake, individual risk factors and medical recommendations all matter, and the evidence around fracture prevention using isolated supplements is more complex than advertising sometimes suggests. [7]

Exercise matters here too: resistance training and weight-bearing activities are useful for musculoskeletal health. [2,6]

Why can weight or waist size seem to change?

Many women notice that clothes fit differently during this stage, even when the change on the scale is not dramatic.

It is worth avoiding the simplistic idea that “menopause destroys your metabolism”.

The reality is more complex.

The menopausal transition is associated with changes in body composition and a tendency towards more abdominal fat distribution. At the same time, ageing, physical activity, sleep, energy intake and muscle mass also play a role, and research is still working to separate the contribution of each factor. [5]

The answer is therefore not automatically to eat much less.

Very restrictive diets can be difficult to maintain and can make it harder to meet your needs for protein, fibre, vitamins and minerals.

It is usually more useful to look at:

portion sizes;

how often very energy-dense foods appear in your day;

how much protein and vegetables you eat;

snacks eaten when you are not actually hungry;

your level of physical activity;

the quality of your sleep.

You do not need to track every calorie obsessively in order to improve the overall structure of your diet.

Fibre matters for more than digestion

Vegetables, fruit, pulses and whole grains provide fibre as well as a wide range of vitamins, minerals and other plant compounds.

Fibre supports normal bowel function and can help with satiety. A diet rich in plant foods also fits well with general recommendations for cardiovascular health. [4]

In practical terms, try to include plant foods at most meals rather than relying on one small salad at dinner.

If you currently eat very little fibre, increase it gradually and make sure you are also drinking enough fluids.

Can some foods make hot flushes worse?

For some women, yes. For others, no.

Alcohol, caffeine, very hot drinks and spicy foods may act as triggers for some people. [8]

But there is no need to remove all of them automatically.

If you drink coffee in the morning and notice no connection with your symptoms, there is no good reason to stop just because you have entered perimenopause.

It is more useful to look for patterns:

“Are my night sweats consistently worse after drinking alcohol in the evening?”

“Does afternoon coffee make my hot flushes worse, or is it mainly affecting my sleep?”

“Does spicy food genuinely trigger my symptoms, or did I remove it simply because I read that I should?”

Adjust what repeatedly causes problems for you rather than turning your diet into a list of unnecessary restrictions.

Sleep and movement matter almost as much as food

When sleep is disrupted for weeks or months, maintaining the same eating and activity routines can become much more difficult.

Hot flushes and night sweats can interfere with sleep, and tiredness can affect the energy you have for exercise and meal planning.

Try to protect a regular sleep routine where possible. If you notice that alcohol or caffeine later in the day affects your sleep, reduce them and see whether it makes a difference.

When it comes to physical activity, cardio is not the only thing that matters.

A combination of:

walking and other forms of aerobic activity;

strength training;

activities that support mobility and balance

is more useful than focusing only on burning calories. [2,6]

Do you need supplements “for hormones”?

Not automatically.

Many supplements are marketed for “hormonal balance”, “hormone detox” or “oestrogen reset”, but these phrases are largely marketing language rather than clearly defined medical goals.

Some products, including certain isoflavone and herbal preparations, have been studied for menopausal symptoms. NICE notes, however, that products differ, the safety and purity of unregulated preparations may be uncertain, and interactions with other medicines are possible. [2]

So “natural” does not automatically mean “effective” or “risk-free”.

Start with food, and discuss supplements with your doctor or pharmacist when there is a clear reason to consider them.

When should you speak to your doctor?

You do not need to wait until symptoms become unbearable.

Speak to your doctor if:

hot flushes or night sweats significantly affect your sleep or everyday life;

mood changes become difficult to manage;

symptoms begin before the age of 45, especially before 40;

you have concerns about your bone health;

changes in your periods or bleeding worry you.

If you have gone 12 months without a period and then experience any vaginal bleeding again — even a small amount or a single episode — it should be medically assessed. [9]

Effective medical options are available for menopausal symptoms, including hormonal and non-hormonal treatments. The right choice depends on your symptoms, medical history, risks and preferences. Nutrition and exercise are important, but they should not be used as a reason to delay effective treatment when it is needed. [2,3]

How can you organise meals more simply during this stage?

When you are trying to think about protein, bones, fibre, weight and energy all at the same time, food can start to feel unnecessarily complicated.

It does not have to be.

A meal can simply be:

protein + vegetables or fruit + an appropriate carbohydrate source + a moderate amount of fat.

If you prefer to start with an organised structure, Hormonal Balance provides a 14-day menu with 3 main meals per day and an optional snack. The menu includes varied sources of protein, fibre, calcium and omega-3 fats, and the plan is available in Basic and Premium versions.

Hormonal Balance is an educational meal plan. It does not treat menopause, does not “regulate” oestrogen levels and does not replace medical treatment when treatment is indicated.

Perimenopause and menopause do not require perfect eating. What matters more is consistency: enough protein, a variety of plant foods, attention to bone and muscle health, regular movement and as few unnecessary food rules as possible.

Your body changes. Your strategy can change with it.

Sources and bibliography

https://www.who.int/news-room/fact-sheets/detail/menopause/

https://www.nice.org.uk/guidance/ng23/chapter/Recommendations

https://academic.oup.com/ejendo/article/193/4/G49/8281862

https://pubmed.ncbi.nlm.nih.gov/38617417/

https://pubmed.ncbi.nlm.nih.gov/40489975/

https://www.who.int/europe/news-room/fact-sheets/item/physical-activity

https://thebms.org.uk/publications/consensus-statements/prevention-and-treatment-of-osteoporosis-in-women/

https://www.nhs.uk/conditions/menopause-and-perimenopause/things-you-can-do/

https://www.nhs.uk/symptoms/post-menopausal-bleeding/

Perimenopause and menopause bring real changes to the body.
