[1–3]

But the medicine is not always the only explanation. When your appetite drops, it is easy to eat less, drink fewer fluids and consume fewer fruits, vegetables or whole grains. All of these changes can contribute to constipation.

Important: This article is for informational and educational purposes only. [1–3] At the same time, a reduced appetite can significantly change the way you eat.

You may have stopped eating some of the fruit, vegetables or whole grains you used to include without even realising it. You may be drinking less because you are not as thirsty, or because you also feel nauseous. Or your meals may have become so small that your overall fibre intake has dropped considerably.

Before looking for a complicated solution, it is worth taking a moment to look at what has changed in your diet since starting treatment.

What to eat for constipation when taking Wegovy, Ozempic or Mounjaro

The first instinct when constipation appears is often to add a lot of fibre: more bran, seeds, pulses or perhaps a fibre supplement.

That is not necessarily the best approach.

Fibre can help, but adding a large amount all at once may cause bloating, gas and discomfort. It is better to increase fibre gradually, giving your body time to adjust. [4,5]

You can start with foods that are easy to include in your usual meals:

oats;

pears, kiwi, berries or prunes;

cooked vegetables;

wholegrain bread and cereals;

lentils, chickpeas or beans, in small portions if you tolerate them;

nuts and seeds.

You do not need to add all of them in one day.

If a large salad or a portion of pulses makes you feel bloated, there is no point forcing yourself to eat them simply because they contain fibre. Try a smaller portion, cooked or roasted vegetables, or a vegetable soup and see how you feel.

Fibre also needs fluids

As you increase fibre, pay attention to how much you drink as well. Fluids help fibre work properly and can make stools easier to pass. [5]

Put simply, a lot of fibre combined with very little fluid can make the problem more difficult rather than easier.

Try to drink gradually throughout the day. Water should remain the main source of hydration, but soups and other fluids that you tolerate well can also contribute.

There is no single amount of fluid that is right for everyone. If you have kidney or heart disease, or you have been advised to restrict your fluid intake, follow the recommendations of your medical team.

What habits can support bowel movements during GLP-1 treatment?

Food matters, but it is not the only thing that affects bowel function.

Regular movement can help. You do not need an intense workout; even a walk may be useful if your health allows it. [5]

Try not to repeatedly ignore the urge to go to the toilet. For some people, allowing a few unhurried minutes after breakfast can help establish a regular routine.

If you are eating very little because you also feel nauseous, it is worth looking at the bigger picture rather than focusing only on constipation. See also Nausea during GLP-1 treatment.

For a broader explanation of meal structure, protein, hydration and digestive symptoms during treatment, read Nutrition During GLP-1 Treatment: What to Eat When Taking Wegovy, Ozempic or Mounjaro.

What if constipation during GLP-1 treatment does not improve?

If you have increased fibre gradually, you are drinking enough fluids and staying active, but the constipation continues, the answer is not necessarily to keep adding more fibre.

There are other options for treating constipation, including different types of laxatives, but they do not all work in the same way and they are not suitable for every situation. If you find yourself needing them repeatedly, speak to your doctor or pharmacist. [5]

You should also speak to your doctor if the symptoms appeared or became noticeably worse after a change in your treatment dose.

Constipation can have many causes, so a persistent problem should not automatically be blamed on GLP-1 treatment.

When should you contact your doctor about constipation?

Mild constipation can often improve with changes to food and daily habits. However, some symptoms should not be ignored.

Seek medical advice if you develop:

severe or persistent abdominal pain;

an inability to pass gas;

vomiting;

blood in your stool or rectal bleeding;

fever.

Constipation that persists or becomes worse despite the usual measures should also be discussed with your doctor, even if none of these warning signs are present. [5]

How to organise meals when your appetite is reduced

When you are eating much less than before, it can become difficult to think about how to include enough protein, fibre and foods that you can actually tolerate.

If you prefer to start from an already organised meal structure, you can look at Metabolic Reset for Women or Metabolic Reset for Men and adapt the meals according to your own tolerance.

The meal plans do not treat constipation and do not replace medical advice.

You do not need to change everything in a single day. Increase fibre gradually, drink fluids throughout the day, keep moving as much as your health allows and notice which foods you tolerate best. Small, consistent changes are often easier to manage than a complete overhaul of your diet.

Sources and bibliography

https://www.ema.europa.eu/en/medicines/human/EPAR/wegovy

https://www.ema.europa.eu/en/medicines/human/EPAR/ozempic

https://www.ema.europa.eu/en/medicines/human/EPAR/mounjaro

https://pubmed.ncbi.nlm.nih.gov/40450457/

https://www.niddk.nih.gov/health-information/digestive-diseases/constipation

Constipation can occur during GLP-1 treatment.
