GLP-1 acid reflux may feel like a burning sensation rising from the stomach towards the chest, a sour taste in the mouth or the feeling that food is coming back up after a meal. Even a small portion may become uncomfortable, particularly in the evening.
When these symptoms begin or become more noticeable after starting treatment, food choices, portion size and meal timing may all matter. However, persistent reflux should not automatically be attributed to the injection and should be discussed with your doctor.
Why can GLP-1 acid reflux occur?
Wegovy and Ozempic contain semaglutide. Mounjaro contains tirzepatide, a dual GIP and GLP-1 receptor agonist. These medicines can affect gastric emptying, and gastro-oesophageal reflux disease or heartburn is listed in their current official product information. [1-3]
For people who are prone to reflux, prolonged fullness, large portions and high-fat meals may be harder to tolerate and may worsen heartburn or regurgitation. Reflux has several possible causes, however, and changes in digestion are only one potential explanation.
What to eat when you have acid reflux
Start with smaller portions
A very large meal may increase the feeling of fullness and make it easier for stomach contents to move back towards the oesophagus.
Try to:
- start with a small portion;
- eat slowly and chew thoroughly;
- stop when you begin to feel full;
- avoid compensating in the evening for food you did not manage to eat earlier in the day.
Smaller portions, slower eating and stopping when fullness develops are commonly recommended when managing digestive symptoms associated with GLP-1 treatment. [4]
Choose simple, lower-fat meals
When reflux is more noticeable, you could try:
- well-cooked rice, potato or porridge oats;
- cooked vegetables, if tolerated;
- lean chicken or turkey;
- white fish;
- eggs prepared with little added oil;
- plain yoghurt or lower-fat cheese, if dairy products do not worsen your symptoms;
- simple soups in moderate portions.
Fat does not need to be removed completely. However, a meal that combines fried food, rich sauces, a large amount of oil, high-fat cheese, avocado and nuts may contain more fat than you tolerate comfortably. Both the amount and the combination of foods matter.
Use gentler cooking methods
Boiling, baking, steaming or cooking in a non-stick pan with a small amount of oil may be easier to tolerate than frying.
Choose a comfortable serving temperature and eat slowly. When a particular meal or temperature repeatedly worsens your discomfort, adapt it to your own tolerance.
Foods that may worsen heartburn
Triggers vary from one person to another. Foods and drinks commonly linked with reflux symptoms include:
- very high-fat meals and fried food;
- spicy food;
- tomatoes and tomato-based sauces;
- citrus fruit;
- chocolate;
- coffee and other sources of caffeine;
- mint;
- alcohol.
Do not remove all of these foods automatically. Notice what happens after each one and reduce only the foods or drinks that repeatedly worsen your symptoms. [5]
A simple food and symptom diary may help. Record what you ate, the amount, the time of the meal and when heartburn developed.
What to do in the evening to reduce reflux
Meal timing and body position may affect symptoms that occur in the evening or when you are lying down.
Try to:
- avoid lying down immediately after eating;
- avoid very large evening meals;
- leave around three hours between your final meal and going to bed;
- notice whether heartburn occurs more often after late or particularly rich meals.
When symptoms occur at night or while lying down, finishing your last meal at least three hours before bed may help. [5]

When to contact your doctor
Speak to your doctor if reflux:
- occurs frequently or persists;
- becomes worse after a dose increase;
- prevents you from eating or sleeping;
- does not improve after changing portion sizes and eating habits;
- is accompanied by persistent vomiting.
Seek prompt medical assessment for chest pain, difficulty or pain when swallowing, vomit containing blood or resembling coffee grounds, or black, tarry stools. These symptoms may have other causes and should not automatically be attributed to reflux or GLP-1 treatment. [6]
Do not change your treatment dose yourself or start medicines for acid reflux without speaking to your doctor or pharmacist.
Simpler meals, with less discomfort
GLP-1 acid reflux may be easier to manage when you reduce portion sizes, limit very high-fat meals, identify personal triggers and avoid lying down after eating.
You do not need to permanently remove half of your usual foods. Keep the foods you tolerate and adjust only those foods or habits that repeatedly worsen your symptoms.
